Understanding the New Aged Care Standards – Part 2: Standard 2 – Assessment & Planning

With the strengthened Quality Standards coming into effect on 1 November 2025, Standard 2 takes centre stage in making sure each person gets care that’s responsive, flexible, and built around their evolving needs.

Why Standard 2 matters

Standard 2 ensures that your care is not a one-off checklist but an ongoing, personalised process, co-created with you and those you trust.

It focuses on:

  • Working in partnership with you and your family or chosen representatives
  • Identifying and addressing your needs, goals and preferences
  • Respecting advance care wishes, including end-of-life decisions if you choose to share them
  • Documenting a clear, accessible Care and Services Plan
  • Reviewing and updating the plan regularly or whenever your needs change
  • Coordinating with other care providers to ensure consistency and continuity

At its heart, this standard supports people to live the life they choose, while receiving the support they need.

How we bring Standard 2 to life at Happy Helpers

  • We start by listening — understanding what’s important to you and how we can support your wellbeing, independence and routines.
  • Your Care and Services Plan is built around your goals — whether it’s staying socially connected, continuing favourite activities like gardening or Tai Chi, or ensuring home visits fit around your schedule.
  • Plans are reviewed regularly, and always updated if your needs change — for example, after a hospital stay or health event.
  • We work closely with GPs, specialists or allied health professionals (with your permission) to ensure your care is coordinated and consistent.
  • If you wish to discuss advance care planning, we do so with sensitivity and care, involving your loved ones where appropriate.

Real-life experience

Mrs James recently began receiving home care support following a hospital stay. During her first assessment, we worked closely with her and her daughter to capture her health needs, cultural preferences, and goals — including her wish to regain confidence in cooking independently. Her personalised Care and Services Plan was developed and shared in easy-to-read print. Regular reviews allowed us to adjust her supports as she regained strength, and her input guided every decision. This partnership approach ensures Mrs James feels empowered, respected, and truly in control of her care.
